FIXED: 1096 print alignment issues A number of issues with uploading to the 1099 Pro Service Bureau have been resolved.

FIXED: Printing of the 1096 paper filing transmittal form has been modified to better place the 'X' in the checkboxes for the transmittal form type.

FIXED: Boxes 3 and 6 were not appearing in the custom query field list for form 1099-C.
